Comprehensive Case Management
Individualized goal planning • Screening for substance use disorder and linkage to treatment • Safety planning and support • Referrals and connection to shelter, treatment, and other community resources
Basic Needs
Hygiene and personal care products • Shower and bathroom facilities • Daily meal service
Groups
Trauma-informed yoga • Art • SELF psycho-education groups • Spiritual support
