NEIGHBOR FOR NEIGHBOR HEAT FUND
ELIGIBILITY WORKSHEET
Are you in need of assistance with your gas bill? The Salvation Army (Buffalo) can assist you!!!!
Eligibility-Applicants must require energy-related assistance. Which ONE of the following conditions is present:
- Applicant must be 55 years of age or older.
- Applicant or household member must be disabled or handicapped and receiving Supplemental Security Income (SSI) or Social Disability. The household member must be a permanent resident in the home.(attach documents)
- Applicant must have an acute medical condition that would be worsened by the absence of heating or cooling. (attach documents)
- Applicant must be receiving unemployment benefits.
- Applicant or household member is a veteran.
If you qualify, you will need to bring the following documents:
- Verification of age, disability, or medical condition
- Household income information.
- Information regarding household expenses (paid receipts within the last 30 days) and additional expenses that are essential.
- A signed release of confidential information or a signed Consent (if the applicant is not present at the time of filing)
- Most recent fuel bill.
- Documentation of HEAP benefits received or HEAP denial form.
- Social security cards for all household members
- Applicant photo of household adults.
