Long Beach Red Shield Membership Scholarship Information

Before you start, you will need:

• Proof of Address (Utility Bill, Rent, Mortgage, Housing Voucher)
• Proof of Income (Supplemental Security Income, Disability or Unemployment Letter, Income Tax, W-2, or 3 Consecutive Paychecks; Bank Statement is Optional)
• Number of People in the Home (Names and D.O.B.)
• Picture ID (If none, 2 of the following are required: Birth Certificate, SSC, or School Registration/ID)
• Contact Information (Name, Email, Phone Number, Address)
